Health Forms for Carroll County Public Schools

Fill out and submit the following CCCTC Parking Registration form.

CCCTC Parking Registration

 Parking Permit is only for CCCTC parking lot.

• CCCTC students are subject to verbal warnings, parking fines and loss of driving privileges for driving violations; such as burnouts, revving of engines, foul language, etc.
• Place permits on driver side, lower left in window.
• No Carpooling during the school day. (7:45-1:45)

In the event that a student decides to withdraw from a program, it is recommended that the student FIRST meet with their counselor to make sure of graduation requirements this decision might affect. Then the following form must be filled out and signed by the parent/legal guardian of that student.
